
The Johnson County Department of Health and Environment is nearing completion (in the next few weeks) of vaccinating those in Phase 2. JCDHE is following the Kansas Department of Health and Environment’s Vaccination Plan.
For planning purposes, JCDHE is asking those in Phase 3, 4, and 5 to indicate their interest in getting the COVID-19 vaccine. By completing the survey, individuals will be notified when vaccine becomes available and the state has moved to your phase with instructions on how to schedule an appointment. Those who have completed a vaccine interest survey for Phases 1 and 2 do not need to complete the survey again. This survey replaces all other JCDHE vaccine interest surveys.

As we move through the phases, anyone in a prior phase can make a vaccine appointment when appointments are available. Completing this survey does not create an appointment and does not create a vaccine.
