The Johnson County Department of Health Community Health Workers program is a free resource that helps clients navigate applications for health care coverage and schedule medical and dental appointments, find resources for food, rent and utility assistance, and provides health education to help you create patterns and behaviors that will keep you healthy. 

Community Health Workers are trusted members of the communities they serve who have an understanding of health systems, chronic conditions, disease prevention and community resources.
