Beat the school exclusion deadline! Get caught up on required and recommended vaccinations for the 2024-2025 school year. This one-day walk-in clinic is for 7th or 11th grade students and any student needing a Hepatitis A vaccine. Immunizations are also available during regular clinic hours.

The clinic will provide Tdap, Hepatitis A, Meningitis and HPV vaccines. Parents can save time by completing the consent form before the clinic (links below). A parent must accompany the student to the clinic. No appointment is needed. 

What to bring:

• Letter from the school indicating which vaccines are needed
• Student vaccination record
• Signed consent form: Immunization Consent Form or Formulario de Consentimiento Para las Vacunas
• Insurance card (if you have one)
