Students entering 7th or 11th grade in August 2024 can get caught up on required school vaccinations at one of six walk-in clinics in June, July and August 2024. 

Each clinic will provide Tdap, Hepatitis A, Meningitis and HPV vaccines. Parents are encouraged to complete paperwork using this online form. A parent must accompany the student to the clinic. No appointment is needed. 

What to bring:

• Signed Consent Form (if not completed online): Immunization Consent Form or Formulario de Consentimiento Para las Vacunas
• Insurance Card (if you have one)
