Health Supports
JCDS Health Supports offers a variety of clinical services and supports necessary for people with disabilities to maintain health. The Clinical Support staff focuses on the health and wellness of individuals supported by JCDS and actively seek to curb the development of serious health concerns by monitoring preventative care for persons supported.
Case Management
The JCDS case management team is comprised of degreed, experienced professionals, who are dedicated to empowering individuals to lead enriched, interdependent, healthy, safe and choice-driven lives.
Wellness Monitoring
Evaluation and regularly scheduled nursing visits by a registered nurse for individuals who have medical conditions that require ongoing oversight.
Targeted Case Management
Case management that is provided by a licensed individual or agency that assists the person and support network.
Supportive Home Care
Supportive Home Care services that are one-on-one assistance for individuals living with family or a foster family.
Supported Employment
Supported Employment provides support services to adults ages 18 and older to secure and maintain competitive, integrated work.
Specialized Medical Care
Provides long-term Registered Nurse or Licensed Practical Nurse support for people who are medically fragile and technology dependent.
Personal Care Services
Personal Care Services allow the person or guardian to self-direct residential supports, day supports or supports in the family home.
Overnight Respite
Overnight Respite is a service that provides temporary direct care and supervision of the person in order to provide relief to families and caregivers.
Medical Alert
Medical Alert provides small devices carried or worn by the person to provide access to assistance or emergency help 24 hours a day.
