Summer Vaccination Clinics
Students entering 7th or 11th grade in Aug. 2024 are encouraged to visit one of these walk-in vaccination clinics to receive recommended vaccinations for the 2024-2025 school year. Some of these vaccinations (Tdap, Hepatitis A and Meningitis) are required by the Kansas Department of Health and Environment to attend school and participate in school activities.
Each clinic will provide Tdap, Hepatitis A, Meningitis and HPV vaccines. Parents are encouraged to complete paperwork using this online form. A parent must accompany the student at the clinic. No appointment is needed.
What to bring:
- Signed Consent Form (if not completed online): Immunization Consent Form or Formulario de Consentimiento Para las Vacunas
- Insurance Card (if you have one)
