Chronic Disease Management

Chronic diseases are a major concern for many adults in the United States, with nearly half of all American adults living with conditions like heart disease, diabetes and obesity. While these challenges can seem overwhelming, managing them is key to living a healthier, happier life. Simple lifestyle changes can make a big difference in preventing complications and improving well-being. By taking charge of their health, individuals can not only feel better but also enjoy a higher quality of life and set a positive example for their families and communities. Embracing proactive management of chronic diseases is a wonderful step toward long-term health and happiness for everyone!

Johnson County Department of Health and Environment offers two programs focused on chronic disease management. 

 

Chronic Disease Self Management Program

About half of adults have one or more chronic health conditions such as diabetes, heart disease, cancer or arthritis. If you have a chronic condition, self-management education can help you develop strategies and confidence to manage your condition day to day. CDSM workshops are a free 6-week program that will help participants gain control of their health. Topics covered include: 

  • Techniques to deal with problems such as frustration, fatigue, pain and isolation
  • Appropriate exercise for maintaining and improving strength, flexibility and endurance
  • Appropriate use of medications
  • Communicating effectively with family, friends and health professionals 
  • Managing depression 
  • Healthy eating habits
  • How to evaluate new treatments

Walk with Ease

The Arthritis Foundation Walk with Ease program is a free exercise program that can reduce pain and improve overall health. If you can be on your feet for 10 minutes without increased pain, you can have success with Walk with Ease. Benefits of Walk with Ease include motivation to exercise, learning how to walk safely and comfortably, improving flexibility, strength and stamina, and reduction of pain and feeling great!
